The Polis – Healthy Smart Cities platform (“Platform”), consists of (1) the Polis – Healthy Smart Cities computer application, (2) the website http://www.saudepolis.pt/ (3) the products, health programs (“Polis Health Program”) and services associated with the application and the website. The Platform is provided by the company HSC – Healthy Smart Cities, Lda., headquartered at Rua das Infestas number 335, 4520-022 Escapães, Santa Maria da Feira, Portugal.
For HSC – Healthy smart Cities, Lda., the respect for the privacy of Users of this Platform (“Users”), its customers, as well as compliance with applicable legal obligations is a priority. We take the all the necessary measures to protect their personal data.
The aim of this privacy policy is to inform our customers of their rights and explain how their personal data is collected and used within the scope of the Platform’s activity.
By using the services provided by HSC – Healthy smart Cities, Lda., and filling in the forms available on the Platform and directly or indirectly providing data, you acknowledge the conditions established in this privacy policy.

HSC – Healthy Smart Cities , Lda. is the entity responsible for processing the personal data in the scope of the Platform, defining the data collected, the means of processing and the purposes for which the data will be used.

Notwithstanding any complaints submitted to the data protection officer, the data subject may also submit their complaints to the Comissão Nacional de Proteção de Dados (CNPD).
- Personal data and collection method
The holders of personal data are the natural persons to whom the personal data relates and who use or intend to use the services provided by the Platform.
3.1 Non-Identifiable Information. The Platform may automatically collect certain non-identifiable information about you, such as the serial number of their digital devices. This non-identifiable information is used to send you electronic communications when necessary. At any time, you can ask us to stop sending you Electronic Communications by contacting saudepolis@saudepolis.pt. Your non-identifiable information may be disclosed to third parties and permanently archived for future use.
HSC – Healthy smart Cities, Lda. may also associate your non-identifiable information with users’ personal information. When such a link is made, all related information is treated as personal information and will be used and disclosed only in accordance with this Policy, and in accordance with the law.
Data relating to health is data related to the health status of the data subject, which reveals information about their physical or mental health, in the past, present or future.
The Platform uses tables, graphs and calculations to allow you to track your entered data over time and to compare your data with standardized, anonymized values from the general population. The Platform is intended for your personal, educational, informational, entertainment and convenience use only.
3.2 Personal Information
Your personal information will be required for the purpose of creating an account protected by username and password. For this, you must provide your e-mail address, your username and a password of your choice. The email address can be used to validate the registration and to communicate or send selected information of general interest or related to healthcare, including newsletters, alerts, notifications.
Registration on the platform requires prior use of a registration code, which will be provided upon admission to the Polis Health Program.
At any time, you can ask us to stop sending Electronic Communications, by contacting saudepolis@saudepolis.pt or using the clickable subscription cancellation at the bottom.
Use of Bluetooth Devices. The Platform can be connected via Bluetooth technology to devices measuring health parameters. The manufacturers of these devices have chosen to use Bluetooth LE (Low Energy) communication as a means of providing a “real-time” experience for users, allowing immediate retrieval of data from their devices. The Platform does not use Bluetooth for any other purpose. If you do not wish to have your personal information transmitted over Bluetooth, you can disable Bluetooth on your smartphone or similar device in the settings app for that device, or it may simply not turn it on at all.
HSC – Healthy Smart Cities, Lda. considers that by joining the Platform, you use it voluntarily and provide us with information which means you are giving your consent for the permitted uses.
HSC – Healthy Smart Cities, Lda. undertakes to process your personal data and in particular your health data, in strict compliance with the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016 (General Regulation on Data Protection or “GDPR”) and Law No. 58/2019, of August 8, which ensures the implementation of the GDPR in the national legal system.
3.3. Anonymized analytical information for health programs. Aggregated and anonymized statistics may be prepared within the framework of the development of the Platform’s health programs. This information is not identifiable by user, and compliance with the provisions of the GDPR is guaranteed.
3.4. Changes to Personal Information. You can update your information by notifying HSC – Healthy smart Cities, Lda. of any changes to your personal information, in writing, to the following email address saudepolis@saudepolis.pt.
3.5. Withdrawing consent. You may withdraw your consent to the use of personal information at any time. To do this, contact HSC – Healthy smart Cities, Lda. to the following email address saudepolis@saudepolis.pt .
- Changes to the Privacy Policy. The privacy policy can be changed by HSC – Healthy smart Cities, Lda. You will be notified of changes to this privacy policy by HSC – Healthy smart Cities, Lda. and you will be asked to review and agree to the new privacy policy.
When you use the Platform, receive or request any electronic communication, you must check the date of this privacy policy and review all changes since the last version.

The Terms and Conditions will be governed by and interpreted in accordance with Portuguese law. For this purpose, the legally binding text is the Portuguese version of this document.
